The use of the facility for a birthday party is requested on the date listed above, between the hours of 10AM and 12PM. By submitting this form you agree to abide with the stated policy and rules of the Dallas Firefighter's Museum, waive any claim against the Dallas Firefighter's Museum for any injuries suffered by reason of any defect in the premises and agree to pay the Dallas Firefighter's Museum for any damages done to said premises.
